When was the last time you had your hair cut and/or colored?

What is the current state of your hair? (health, length, color)

Have you ever used over the counter box dye?

What is your ultimate hair goal? (desired color and cut)

Is there anything else we should know? For example, did you have a preferred stylist? Were you hoping to be in our PA or DE location?

Please also send a few pictures of your current hair in natural lighting - photos of the front, back, and sides. Also, if possible, inspiration photos (Required).
